Recording your decisions in a crisis or incident is absolutely vital and so often missed. If something were to go wrong, your decisions will be subject to extensive scrutiny, very often many years later and you will be expected to clearly articulate why you acted as you did. If you do not have an accurate record to fall back on, clearly explaining the information you had available at the time and the reason you made the choice you did, others will form their own opinion with the luxury of hindsight.
